Participation Agreement

Welcome to LittleSteps4Health! This is a free 6-week at-home program to support children and families in making healthier choices around food and exercise. Currently, the project is in the pilot phase and we ask that families submit their honest feedback throughout the program. Our goal is to provide tips and tricks to help you create a healthier lifestyle by taking those little steps!

We provide FUN interactive materials for you and your family, covering topics such as dealing with picky eaters, creating a balanced diet, setting goals, and so much more! Additionally, connect with other families and EVMS staff through our online support group. At the end of each week, our staff will call for a 10-minute follow-up via telephone or Zoom for a quick check-in on any improvements. At the very end of the program, our staff will do a final wrap-up call lasting no longer than 20-30 minutes for final feedback. Additionally, we can let your child's physician know about their completion of the program.

You and your family are expected to complete each week's worksheets and coordinate a time with our staff for a weekly follow-up. This program is free and will not be billed to your insurance provider. Participation in this program is entirely voluntary and you can stop your participation at any time. If you choose to withdraw from the program, we ask that you contact the Program Coordinator/Assistant at cinch@evms.com or 757-446-6039 and complete a brief survey with our staff regarding your withdrawal, so that we can better understand your reasons for dropping out.
